Athlete Registration
Step By Step Instructions
Step 1: Register Online
Go to https://usaboxing.webpoint.us/ then click on "Join USA Boxing"
Fill out the required information.
*** Make sure to click on Athlete Membership and check mark the Boxer Box ***
At the end, you will be asked to pay the USA Boxing Membership Fee
Step 3: Upload Your Physical
There are three pages of the Annual Athlete Physical Form that needs to be completed in order to receive your Athlete's Passbook. These must be signed by a certified physician, and uploaded to your Online USA Boxing Account. Step-By-Step Instructions are below.
Athlete's Physical Upload Instructions
***YOU WILL NEED TO RETAKE YOUR PHYSICAL WHEN IT HAS EXPIRED AFTER 1 YEAR. IF YOU SHOW UP TO A BOXING EVENT WITH AN EXPIRED PHYSICAL YOU WILL BE UNABLE TO COMPETE!!!***
Boxers who are 41+ years old will instead have to complete and upload the Masters Physical Examination Form
Step 3: Recieve your passbook
Articles Needed to Receive Athlete's Passbook

2 (2x2) Sized Photos of Athlete

2 Copies of USA Boxing Membership Confirmation Page

2 Copies of the Annual Athlete Physical Form (And virtually uploaded)

Copy of Athlete's Birth Certificate or Passport
How to print out USA Boxing Membership ID Card
If any of the below forms are applicable to the athlete, you are required to complete, print, and provide them to our Registration Chair in order to receive your passbook.

When all of the materials are gathered, contact our Registration Chair to schedule an appointment to receive your Athlete's Passbook
